Audio/Visual Permission
Staff member gives permission for AotS to reproduce through the following, the image or voice of the staff member during their participation in the Event:
- The use of still photography in Autism on the Seas promotional printed literature.
- The use of video, voice or still photography in Autism on the Seas television, radio, webinar, website, Facebook page, or any internet related medium.
- The complete confidentiality of all audiovisual materials will be maintained and that these materials will only be used for the purposes indicated above.
Release of Liability
Know Ye, That I, Staff Member, hereinafter designated as the Releasor, does remise, release and forever discharge the release Autism on the Seas, Michael Sobbell, Autism on the Seas, it’s agents, servants, cruise lines, vacation supplier, employees, and volunteers from all debts, obligations, reckonings, promises, covenants, agreements, contracts, endorsements, bonds, specialties, controversies, suits, actions, causes of actions, trespasses, variances, judgments, extents, executions, damages, claims or demands, in law or in equity, which against the said Releasee, the Releasor ever had, now has or hereafter can, shall, or may have, for, upon or by reason of any matter, cause or thing whatsoever, from the beginning of the world to the day of the date of these Presents. More particularly, for any and all claims against Autism on the Seas, it’s agents, servants, employees, volunteers, cruise lines, vacation supplier, or any other individuals associated with Autism on the Seas during this Cruise. Wherever in this instrument any party shall be designated or referred to by name or general reference, such designation is intended to and shall have the same effect as if the words "heirs, executors, administrators, personal or legal representatives, successors and assigns" had been inserted after each and every such designation and all the terms, covenants and conditions herein contained shall be for and shall inure to the benefit of and shall bind the respective parties hereto, and their heirs, executors, administrators, personal or legal representatives, successors and assigns, respectively. In all references herein to any parties, persons, entities or corporations the use of any particular gender or the plural or singular number is intended to include the appropriate gender or number as the text of the within instrument may require.
Communicable Diseases - In consideration participating in the Autism on the Seas functions, events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, norovirus, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,
2. KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
3. Willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Autism on the Seas officers, officials, agents, volunteers, and/or employees, other participants, any person that we may come into contact with, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
Staff Information
Staff member understands and agrees that all information furnished to AotS may be shared by Autism on the Seas employees or representatives, and/or event personnel. Email addresses and cell phone numbers will be shared with all Staff Members for purposes of communicating prior to the event.
